      Hon. Eran Wickramaratne,— To ask the Minister of Health,—

      (a) Will he state  on per year basis, the number of dengue cases,—

      (i) reported in Sri Lanka since 2012 to-date; and

      (ii) resulting in deaths reported from 2011 to-date?

      (b) Will he also state  the number of —

      (i) dengue breeding grounds detected; and

      (ii) schools, universities and state  institutions that have been identified as containing dengue breeding grounds separately;

      on per year basis from January 2009 to-date?

      (c) Will he inform this House from January 2010 to-date on  per year basis—

      (i) the three districts in Sri Lanka, from which the highest number of dengue cases were reported; and

      (ii) the number of breeding grounds detected in each of these three districts?

      (d) Will he further state—

      (i) the committees that have been appointed by the Ministry of Health to address the dengue epidemic in the country, from 2009 to date;

      (ii) the dates of appointment of each of those committees; and

      (iii) the mandate of each of those committees?

      (e) If not, why?
