Hon. Ajith Mannapperuma,— To ask the Prime Minister and Minister of Buddha Sasana and Religious Affairs,—
(a) Will he inform this House—
(i) whether an X-ray machine produced in Japan was purchased in the year 2013 for the hospital of Sri Lanka Navy in Welisara;
(ii) if so, of the date on which it was purchased;
(iii) of the amount spent on it;
(iv) of the name of the Company which imported it;
(v) whether that machine was subjected to a quality check and approval was obtained prior to receiving it for the Navy hospital;
(vi) if so, the name and designation of the persons who granted that approval;
(vii) whether Sri Lanka Navy and the relevant company had entered into an agreement with respect to a guarantee for the X-ray machine and its functioning; and
(viii) if so, of the relevant warranty period?
(b) Will he state—
(i) whether the said machine is in working condition at present;
(ii) if not, the date on which it became inoperative; and
(iii) if that machine has become inoperative within the warranty period, whether it is possible to get it repaired by the relevant company free of charge?
(c) Will he also state—
(i) whether some irregularity has occurred in purchasing that machine; and
(ii) if so, the action taken in that regard?
(d) If not, why?
