Hon. Tissa Attanayake,— To ask the Minister of Health,—
(a) Is he aware that a sum of rupees 5000/= has been collected from Family Health Officers who serve in the MOH offices through the Family Health Bureau in order to offer a gift to the President to commemorate the "Mid Wifery Day" which fell on 12.05 2014?
(b) Will he state—
(i) the number of MOH offices situated island wide;
(ii) the total number of Family Health Officers serving in those offices;
(iii) the total amount of money charged from the Family Health Officers as mentioned above;
(iv) the gift which has been offered to the President with the aforesaid collection; and
(v) the reasons that contributed towards offering such a gift by the Family Health Officers of the State Family Health Services Union?
(c) Will he inform this House—
(i) of the name of the chairperson of the State Family Health Services Union; and
(ii) of the qualifications possessed by her?
(d) If not, why?
